75 is not old these days and it is important that we do all we can to maintain a patient’s quality of life, dignity and independence. This assessment is really most effective when it is undertaken by your regular GP who is familiar with your history over the preceding 12 months.
The initial part of the assessment is carried out in the patient’s own home by a nurse from EPiC Health Solutions employed by the Practice specifically for this. She will ask about how you are coping at home to identify any areas where a little extra help might be useful such as going shopping or gardening or perhaps you need help with showering or organising your medications. We have knowledge of what help is available to you and can help put you in touch with the right people.
The second part of the assessment is carried out at the surgery when Dr Bartels will discuss your physical, psychological and social well being and whether any preventative measures are necessary. Regular health checks like this are valuable for you and your GP in identifying any needs or concerns you might have about maintaining your independence in your own home.
